All DayJams Parents must sign off on the tuition, cancellation, camp insurance, and photo policies. PLEASE RETURN THIS FORM TO DAYJAMS MAIN OFFICE 2 WEEKS BEFORE CAMP STARTS.

Massachusetts requires your child to have a physical every two years and up to date immunizations. You should obtain a physician form directly from your child’s physician’s office and send it to DayJams along with the Authorization for Treatment Form. If your child needs medication during camp, including epipen & inhalers, please fill out the Authorization To Administer Medication To A Camper.

Please bring a copy of immunizations along with the Authorization for Treatment and 2012 Policies forms to camp for the first day.
If your child needs medication during camp, including epipen & inhalers, please fill out the Authorization To Administer Medication To A Camper and let the DayJams office know 2 WEEKS BEFORE camp starts by calling 800-295-5956.
