2008 Registration Form

Release Form

Parents must read, understand, and sign our Release Form agreeing to our: Tuition Policy, Cancellation Policy, and optional Photo, Video, Release Policy.  Parents AND Campers must read and agree to our Camper Code of Conduct.

One form covers all of the above, and your signed Release Form must be returned to DayJams in order for your child to attend camp.  If you have any questions please give us a call: 800-295-5956.

Scholarship Form

Thanks to our sponsors we have scholarship funds available for campers in need of financial assistance. Please fill out the form and submit to DayJams for consideration.  Any questions call: Crystal @ 800-295-5956  ext.112

Health Forms

One form must be completed by a physician and the other by a parent. In lieu of the Physician Health Form a copy of a current school or annual physical form will be accepted.
