In order for your child to attend camp you must submit the following three (3) forms to the DayJams CT office prior to your child's first day of camp:
1.) Parent Health Form ( must be signed by a Parent or Legal Guardians).
2.) Physician Health Form ( a copy of a current physical form signed by a licensed physician also accepted).
3.) Release Form: Parents must read, understand, and agree to our Cancellation Policy,
Tuition Policy, and Photo, Video, Release Policy. On the same Release Form, Parent
AND Camper must agree to and sign Camper Code of Conduct. One form covers it all.
FINANCIAL ASSISTANCE
Anyone in need of financial assistance is encouraged to submit a DayJams Scholarship Application. Partial to full scholarships will be awarded based on need, desire to attend, and/or musical ability.
